VARSITY & JV ELIGIBILITY REQUIREMENT: Each Varsity and JV team must be properly sanctioned by the high school it represents. Upon registration, or at the very latest, upon arrival at the tournament, each VARSITY & JV team's advisor shall present a letter on it's school's stationery, signed by the principal or activities director, listing each player's name and stating each player's status--how s/he is qualified to represent the school according to the OHSCTA bylaws. (For additional information about tournament eligibility, please read the OHSCTA Bylaws.) This requirement is only for Varsity and JV teams.

I have read and understand the OHSCTA Bylaws. My teams/individuals qualify and are eligible to play.
I understand that I must provide the OHSCTA with an eligibility letter on high school stationery, listing all eligible students, signed by the principal or athletic director, on or before the tournament date, in order for my Varsity or JV team to participate in the State Championship.
I have read and discussed the section on team etiquette with my team members and they understand the rules of the tournament.

Team Tournament early registration discount (by Feb 17th): $100.00 ($20/player)
New/Returning Varsity and JV Teams: $50.00. Note: This is a special discount to attract new Varsity and JV teams to OHSCTA and to encourage Varsity and JV teams that have been inactive in the OHSCTA championships for at least the last three years to return.

Registrations for all teams and individuals received AFTER February 17th:
$120.00 per team ($24/player)

NO ONSITE REGISTRATIONS! YOU MUST PRE-REGISTER BY March 1st TO ATTEND.

Payment may be made in advance or at the door.

Step 2: Print out this form on your school letterhead (or attach a separate letter)
Step 3: Obtain appropriate signature
Step 4: Make check payable to OHSCTA
